Britain's justice system is in the firing line again after it emerged the prison service was unaware for six days that an Algerian sex offending migrant had been accidentally freed.
Brahim Kaddour-Cherif, 24, was released by mistake from HMP Wandsworth in London on October 29.
It is understood that the Algerian migrant was allowed out due to a serious communication error within the prison service.
Authorities only realised Kaddour-Cherif had been mistakenly released when he was due to appear in court on Tuesday but could not be located.
And a convicted fraudster, identified by Surrey Police as 35-year-old William Smith, was mistakenly released on November 3, just five days after Kaddour-Cherif - and shortly after he was handed a 45-month prison sentence.
Multiple police manhunts are now underway - but David Lammy on Wednesday accused the Conservatives of turning Britain's justice system into a "shameful spectacle".
The Ministry of Justice says: "Releases in error have been increasing for several years and are another symptom of a justice system crisis inherited by this Government.
“We have introduced mandatory, stronger prisoner release checks to keep our streets safe and protect the public as well as investing record amounts into our courts – including to improve operational assurance.
"But we are clear that these mistakes must not continue to happen, which is why the Deputy Prime Minister David Lammy announced an independent investigation to look at the serious issue of releases in error across the prison estate."
